We are hiring for a full time Case Manager in Salt Lake City, Utah!!
This Case Manager role of the permanent supportive housing case manager is to provide supportive services for clients residing in permanent housing at First Step House facilities. Permanent supportive housing PSH is a housing voucher program for individuals with disabling conditions that were previously homeless or at risk of homelessness. These vouchers are project based at specific housing locations managed by First Step House in partnership with community housing authorities. Responsibilities include assisting with the eligibility process, conducting comprehensive needs assessments, developing individualized service plans, providing case management services that assist clients in maintaining housing, and helping clients work towards self-sufficiency. The goal of the permanent supportive housing is to utilize a recovery oriented, housing first philosophy in providing rental assistance and supportive services to prevent returns to homelessness. The permanent supportive housing case manager assists in other administrative tasks as needed and works in concert with the priorities of community housing authorities as well as the Executive Director, Executive Team, and the Board of Directors of First Step House.

- Pay Rate $43,680 per year
- Monday - Friday flexible hours

Education and Experience
- Case management experience is required
- An active license as a Social Service Worker in the State of Utah is preferred
- A minimum of a Bachelor's degree in social work or related field is preferred
- Experience in the substance-abuse field is preferred
Duties include but not limited to:
- Identify and/or receive households slated for housing assistance and assist with eligibility for the program.
- Enroll eligible individuals in HMIS.
- Assist with referrals and initial application processes.
- Assist housing authority staff in completing move-in paperwork and lease signing.
- Assist clients with moving into subsidized unit. Utilize community resources to secure household items needed.
- Perform comprehensive needs assessments.
- Develop and follow an individualized service plan with clients to help ensure housing stability.
- Provide ongoing case management and service coordination, including regular home visits.
- Provide client advocacy when needed.
- Perform regular, individual case-management sessions.
- Intervene if a client's behavior is jeopardizing housing stability or threatens the health or safety of themselves or others.
- Collaborate and consult with supervisors, treatment-team members, community partners, and other relevant parties concerning client issues
