Description

First Step House is hiring for a full time Case Manager in Salt Lake City, Utah!!

The Point case manager serves clients who live at The Point, a supportive housing project, by providing a range of services to help them stabilize at the facility and prevent them from becoming homeless. The target population served by The Point includes individuals and couples aged 55 and older with a history of homelessness, disability status, military service, low income, and other criteria in which an applicant may be identified as part of a vulnerable population. This position involves both direct and indirect services including goal setting, needs assessments, tracking progress, and connecting clients with a range of resources e.g., housing, education, employment and medical. The Point case manager assists in other administrative tasks and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors. The overall goals of this position are to a provide high-quality care for clients, b coordinate participation in on site services and programs for those staying at The Point c provide support and coordination with The Point staff.

  • Pay Rate $43,680 per year
  • Monday - Friday flexible hours
  • 36-hour work week

Requirements:

  • Must have a valid drivers license and reliable transportation

Education and Experience

  • Case management experience is required
  • An active license as a Social Service Worker in the State of Utah is preferred
  • A minimum of a Bachelor's degree in social work or related field is preferred
  • Experience in the substance-abuse field is preferred

Duties include but not limited to:

  • Screens clients for program eligibility
  • Orients clients to the community living and activities programs, requirements and consequences
  • Assist clients in applying for eligible entitlement programs
  • Assures that basic nutrition, general medical and behavioral health and hygiene needs of clients are being met
  • Performs comprehensive needs assessments and acts upon critical needs appropriately
  • Develops client driven case management plans and meets regularly to review progress
  • Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
  • Collaborates with The Point staff, case workers, and outside agencies in the delivery of case management services
  • Gathers and sends outcomes and progress reports to grantors and other relevant parties
  • Performs weekly individual case-management sessions or check-ins
  • Conducts monthly wellness checks for all residents
