We are hiring for a full time Case Manager in Salt Lake City, Utah!!
The Orientation Case Manager reports directly to the Treatment Case Manager Lead and then to the Case Management Supervisor and HR Director.
The Orientation Case Manager serves client that are entering into residential treatment services and in the "O hall'. This position helps to orient clients to treatment and case management services, coordinates to complete any follow up items for eligibility for benefits and connects clients with a range of resources (e.g., housing, education, employment and medical). The orientation case manager coordinates with Admissions, Medical Team and Program Manager to provide wrap around services to clients in orientation. They assist in other administrative tasks and works in concert with the priorities of the Executive Director, the Senior Management Team, and the Board of Directors. The overall goals of this position are to (a) provide high-quality care for clients, (b) orient new residents for treatment and case management services and (c) provide support for the clinical and administrative staff.

- Pay Rate $45,445 per year
- Monday - Friday flexible hours

Education and Experience
- Case management experience is preferred
- A minimum of a Bachelor's degree in social work or related field is preferred
- Experience in the substance-abuse field is preferred
Duties include but not limited to:
- Facilitate an orientation group for case management services for O-hall clients
- Perform a condensed needs assessments to help identify urgent needs
- Collaborate with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
- Coordinate closely with ARCAPP and billing to collect and submit any follow up documentation for Medicaid and SNAP eligibility
- Transports clients to pick up necessary belongings and obtain identifying documents
- Address active warrants and schedules arraignments
- Refer clients to dental services
- Collaborates with counselors, case workers, and outside agencies in the delivery of case management services
- Performs regular individual case-management sessions
- Uses internal tracking system to ensure proper, accurate, and timely billing
- Uses the KIPU database to enter case-management notes, client information updates, billing information and other relevant client information
